Trans Abstract

Purpose

Gynecomastia is the most common cause of male breast enlargement and is a disease which can occur in men of all ages. The purpose of this study was to evaluate the effect of tamoxifen on size reduction of breast tissue in gynecomastia patients, regardless of age and symptoms. We also, aimed to investigate the advantages and disadvantages of tamoxifen compared to surgery and other medications.

Methods

Twenty patients diagnosed with gynecomastia at the Breast and Thyroid Surgery Department, of Wonkwang University Hospital from January 2013 to February 2019 were included. All patients received tamoxifen 20 mg once a day or 10 mg twice a day; two months of treatment was planned, and medication was administered. The diameter and depth of the gynecomastia tissue were followed up with breast ultrasound before and after treatment. A survey of age, nationality, location, symptoms, duration of symptoms, previous history, and causes was also conducted.

Results

Breast tissue size was measured by ultrasound when the gynecomastia patients first visited and after using tamoxifen. As a result, mean diameter decreased by 41.9%, mean thickness decreased by 41.0%, and estimated mean volume decreased by 74.3%, except for one patient with recurrence. No side effects of tamoxifen were found in this study.

Conclusion

Since tamoxifen treatment has few side effects and recurrence is rarely reported at the time of discontinuation, it is a good primary treatment. But secondary treatment should be considered if there is no subsequent reaction. Surgical treatment may also be considered for gynecomastia patients with Tanner’s breast stage higher than 4, or who want an immediate reduction in breast size.

서 론

여성형유방은 남성 유방 비대의 가장 흔한 원인으로, 모든 연령의 남성에서 나타날 수 있는 질환이다. 이는 사춘기와 고령에서 주로 나타나며 보통 가역적인 양성질환이다. 편측성 또는 양측성으로 발생할 수 있으며 통증과 같은 증상을 호소하기도 하며 무증상인 경우도 있다. Eberlein [1]에 의하면 그 빈도는 3%–40%까지 다양하게 보고되고 있다.

여성형유방의 치료 방법은 크게 수술적 치료와 약물 치료로 나눌 수 있고, 사용할 수 있는 약물로는 안드로겐(androgen), 항에스트로겐(antiestrogen), 아로마타아제 억제제(Aromatase inhibitors) 등이 있다[2].

본 연구는 연령, 증상의 유무와 관계없이 여성형유방 환자에서 항에스트로겐 약물의 일종인 타목시펜(tamoxifen)을 사용하였을 때 유방 조직의 크기 감소에 대한 효과를 분석함으로써, 여성형유방에 대한 타목시펜의 효과를 입증하고자 하였다. 또한 타목시펜 치료가 수술적 치료 및 다른 약물 치료와 비교하여 어떠한 장단점을 가지는지에 대해서 알아보고자 하였다. 저자들은 여성형유방의 치료제로서 타목시펜을 사용하여 좋은 결과가 있었기에 문헌 고찰과 함께 보고하는 바이다.

방 법

본 연구는 후향적 연구로 진행되었으며 의무기록을 통한 환자의 정보 수집을 시행하였고, Institutional Review Board (IRB No. WKUH 2019-07-012) 승인을 받은 연구이다. 2013년 1월부터 2019년 2월까지 원광대학교병원 유방, 갑상선외과에서 여성형유방으로 진단받은 환자 20명을 대상으로 하였다. 모든 대상환자는 타목시펜 20 mg 하루 한 번 혹은 10 mg을 하루 두 번 투여하였고, 2개월의 치료를 계획하고 약물치료를 시행하였다. 환자들이 여성형유방을 진단받은 후부터 타목시펜을 사용하였고, 1개월에서 6개월까지 치료를 시행하였다. 2개월 사용한 경우가 전체 환자 20명 중 11명(55%)으로 가장 많았으며, 2개월 이상 6개월 미만 사용한 경우는 20명 중 9명(45%)이었고, 6개월 이상 사용한 경우는 없었다. 증상이 2달 이상 지속되는 경우 증상이 호전될 때까지 치료하였고 환자의 증상이 소실된 후 약물을 중단하였다. 치료 기간 동안 환자에게 나타나는 부작용에 대해 추적 관찰하였다. 치료 전후 유방 초음파를 이용하여 여성형유방 조직의 직경과 두께를 추적 관찰하였다. 조직의 직경(장경)과 두께(단경)를 측정하였고 부피=가로×세로×높이 라는 공식을 통해 용적을 추정, 비교하였다. 나이, 국적, 위치, 증상, 증상의 지속기간, 과거력, 원인에 대한 조사를 함께 시행하였다.

결 과

원인과 나이 및 임상 양상에 따른 환자 분포

연구 대상자는 총 20명이었으며, 대상자 중 18명은 한국인이었고 2명은 미국인이었다. 2명의 미국인은 모두 20대였으며 특발성 원인이었다. 연령분포는 20–49세 4명, 50–69세 6명, 70세 이상 10명이었다. 원인은 특발성이 12명, 약물 원인이 6명, 동반 전신질환이 2명이었다. 약물 원인으로 dutasteride 3예, 칼슘통로차단제(calcium-channel blocker) 1예, 양성자펌프 억제제(Proton pump inhibitor) 1예, pregabalin 1예가 있었다. 동반 전신질환이 원인인 경우는 2명으로, 만성신장질환, 폐암이 있었다(Table 1).

임상 양상으로는 양측성 75% (15예), 편측성 25% (우측 2예, 좌측 3예)이었다. 통증이 동반된 경우는 65% (13예), 통증이 동반되지 않는 경우는 35% (7예)로 통증이 동반된 경우가 더 많았으며 증상은 대부분 병원에 내원하기 전 6개월 미만으로 지속되었다(Table 1).

타목시펜의 효과 및 부작용

통증이 있던 환자 13명 중 7명은 1개월 후, 6명은 2개월 후 통증이 소실되었다. 또한 통증이 없이 여성형유방만 있었던 환자 7명 중 6명은 1개월 후, 1명은 2개월 후부터 종양의 크기가 감소하였다.

타목시펜의 부작용으로는 구역, 구토, 피부 발진, 간독성, 백혈구 감소증, 혈소판 감소증 등이 나타날 수 있는 것으로 알려져 있으나, 이는 가역적이며 본 연구를 비롯하여 다른 연구들에서도 이러한 부작용들은 발견할 수 없었다[3-5].

타목시펜의 여성형유방 크기 감소 효과

여성형유방 환자들이 처음 내원하였을 때와 타목시펜을 사용한 이후 조직의 크기를 초음파를 이용하여 측정하였다(Figure 1). 그 결과 재발한 환자 1명을 제외한 나머지 환자들은 평균 직경이 41.9 (±0.093)%, 평균 두께가 41.0 (±0.169)% 감소하였고, 추정 용적은 평균 74.3 (±30.855)% 감소한 것으로 나타났다(Table 2).

고 찰

여성형유방은 남성 유방 비대의 가장 흔한 원인으로, 모든 연령의 남성에서 나타날 수 있는 양성 질환이다. Nuttall [6]에 의하면 남성의 여성형유방의 진단 기준으로는 1) 성인에서 2 cm 이상의 유방조직(더 작을 경우 정확한 측정이 어려움), 2) 유두 범위를 넘어가는 유방조직(최소 발견 가능 직경 0.5 cm), 3) 여성 청소년에서 사용되는 Tanner’s breast stage 3와 비슷한 유방 조직 양상(평균 유두 지름 3–4 mm, 유방 조직은 솟아 있으나 유륜과 유방조직은 하나의 경계선을 가짐)이 있다. Tanner’s breast stage에 대해서는 Table 3에 기술하였다[7].

여성형유방의 원인은 다양하다. 발생 빈도에 따라 크게 특발성(25%)과 사춘기 여성형유방(25%), 약물(10%–20%), 그리고 그 외 원인으로 나눌 수 있다[8].

특발성은 정확한 원인을 감별할 수 없는 경우이며, 사춘기 여성형유방은 성 호르몬 불균형으로 테스토스테론(testosterone)이 감소하고 에스트로겐(estrogen)이 증가하여 발생한다. 여성형유방의 다른 원인 질환으로는 고환부전, 간기능 부전, 갑상선 항진증, 신부전, 당뇨, 비만 등이 있다. 여러 원인 질환들이 있지만 결론적으로 성호르몬의 불균형과 관계된 질환 및 상태에서 여성형유방이 발생하는 원인은 테스토스테론의 감소 또는 에스트로겐의 증가, 에스트로겐/테스토스테론 비 증가로 인하여 유방조직이 비대해지는 것이라고 설명될 수 있다. Choi와 Lee [3]에 따르면 이는 여성호르몬 효과(estrogen effect)라고 할 수 있다. 여성형유방을 유발하는 약물 원인은 다양하다(Table 4) [9-16].

Drug-induced causes of gynecomastia

임상적으로 편측성 또는 양측성으로 나타나며, 환자들은 주로 미관상의 이유 또는 유방의 통증으로 병원에 내원한다. 경과는 자연 소실될 수도 있고, 원인 제거에 의해 소실될 수도 있지만, 미용상 크기가 크고 통증 등의 증상이 있을 경우에는 치료가 요구된다.

여성형유방의 원인이 분명하거나 약물로 인한 경우 원인 질환을 치료하거나 약물을 중단하면 증상이 호전될 수 있다. 그러나 원인 질환의 치료나 약물 중단이 불가능한 경우에는 여성형유방에 대한 치료를 고려해야 한다[17]. 또한 여성형유방이 발생한 지 1년 이상 지난 경우에는 자연적인 소실을 기대하기 어렵다[17,18].

여성형유방은 수술 및 여러 약물로 치료될 수 있다. 증상이 있는 여성형유방에서 전통적으로 수술적 치료를 시행해 왔으나, 수술 후 합병증으로 장액종, 혈종이 흔하고 감염, 피부손상, 반흔, 피부 착색, 유두 소실 등이 발생할 수 있다[19].

본 원에서의 경험으로 볼 때, 비교적 젊은 나이에서 수술적 치료를 더 선호하는 경향을 보였다. 이는 군입대 등의 특수한 상황이 영향을 미친 것으로 보인다. 수술적 치료를 통하여 여성형유방은 즉각적인 호전을 보였으나, 유륜 절개 등 수술로 인한 피부 반흔, 수술 후 발생한 장액종으로 인한 불편감이 있었다. 수술적 치료의 단점에도 불구하고 여러 약물 치료에 반응하지 않거나 악화되는 경우, 또는 환자가 즉각적인 치료 효과를 원하는 경우에는 이 또한 좋은 치료의 하나로 생각된다.

여성형유방의 약물 치료는 1) 안드로겐, 2) 항에스트로겐, 3) 아로마타아제 억제제로 나눌 수 있다[2].

안드로겐 중 테스토스테론은 생식샘 저하증(hypogonadism)으로 인한 여성형유방의 치료로 사용될 수 있으나 테스토스테론이 에스트라디올(estradiol)로 전환되며 부작용을 야기할 수 있기 때문에 사용에 제한이 있다[20].

디히드로테스토스테론(dihydrotestosterone)은 비방향화 안드로겐(non-aromatizable androgen)으로, 사춘기 여성형유방에 좋은 효과를 보이나 근육주사 또는 경피주사로만 사용이 가능한 제한이 있다[21]. 다나졸(danazol)은 생식샘 자극호르몬(gonadotropin) 분비를 저해하는 약한 안드로겐으로, 혈청 테스토스테론을 감소시켜 여성형유방을 치료하는 효과가 있으나 부종, 여드름, 근육경련 등의 부작용이 나타날 수 있다[21,22].

항에스트로겐 중 클로미펜시트르산염(clomiphene citrate)은 에스트로겐 활성도를 약화시키는 비스테로이드제(non-steroidal agent)로, Plourde 등[23]에 의하면 여성형유방의 치료에 효과적이지 못한 것으로 보고되었다. 타목시펜은 선택적 에스트로겐 수용체 조절인자(selective estrogen receptor modulator)로, 에스트로겐 수용체를 선택적으로 차단하는 기전으로 유방에서 에스트로겐의 사용을 저해하고 테스토스테론의 분비를 증가시키는 것으로 여성형유방의 치료에 좋은 효과가 있다[4]. Ting 등[24]에 의하면, 여성형유방 환자에서 타목시펜과 다나졸 치료를 비교하였을 때 타목시펜은 78%, 다나졸은 40%가 효과가 있었다고 보고하였다.

아로마타아제 억제제인 letrozole, anastrozole은 치료 효과가 있을 가능성은 있으나, 최근 연구에서 그 효과가 입증되지는 않았다[25-27].

여성형유방의 치료에 여러 약물을 사용할 수 있겠으나 그중 가장 효과적이고 부작용이 적은 것은 타목시펜으로 생각된다. 이전의 연구에서 타목시펜을 이용한 여성형유방의 치료의 성공률은 약 80%로 보고되었다[4]. 다른 연구들에서 타목시펜을 하루에 10 mg, 20 mg 또는 40 mg, 1개월에서 3개월까지 사용하였고[9,19], 타목시펜을 10 mg을 하루에 두 번 사용하게 되면 대부분 1개월 이내에 통증과 크기가 감소한다고 보고된 바 있다[5]. 이에 본 저자들은 이전의 연구 및 본 원에서의 경험에 비추어 볼 때 타목시펜 20 mg (10 mg을 하루에 두 번 또는 20 mg을 하루에 한 번)을 2개월간 사용하는 방법을 추천한다. 약물의 사용 기간은 환자의 증상이 소실될 때까지 추적 관찰하며 늘릴 수 있다[4].

본 연구는 소수의 환자를 대상으로 한 점, 후향적 연구 방법을 사용한 점, 대조군이 없는 점, 단순한 기술통계 등의 제한점이 있다. 그러나 다양한 연령군에서 서로 다른 원인으로 발생한 여성형유방의 치료에 있어 타목시펜의 효과를 입증하고 치료의 방향성을 제시할 수 있었던 것에 본 연구의 의의가 있다고 하겠다.

전통적으로 여성형유방의 완전한 치료는 수술적 방법에 있으나, 수술적 치료의 시기는 환자에게 선택하도록 하는 것이 중요하다. 일반적으로 수술의 적응증은 심한 통증이 있는 경우, 일상 생활에 지장을 주는 경우, 내과적 치료에 반응이 없는 경우에 해당한다.

수술의 적응증이 아니라면, 타목시펜을 사용한 약물치료의 부작용이 거의 없고 중단 시 재발률도 드물게 보고되기 때문에 일차적인 치료로 타목시펜을 사용하고 이후 반응이 없을 경우 이차적으로 수술적 치료를 고려해 보는 것이 좋은 치료라고 생각된다. 그러나 환자가 즉각적인 유방 크기의 감소효과를 원하는 경우, 여성형유방이 Tanner’s breast stage 4 이상인 경우에는 타목시펜 사용 이전에 수술적 치료를 고려해 볼 수 있다.
